I, hereby acknowledge that I am voluntarily participating in activities organized by Ty Long Kicking. I understand that these activities may involve physical exertion, including but not limited to, football training on the field and workouts in the weight room.
I acknowledge and understand that there are inherent risks associated with participating in these activities, including the risk of injury. I hereby assume all risks of injury, harm, or damage to myself or my property that may arise from my participation in activities organized by Ty Long Kicking.
In consideration of being permitted to participate in these activities, I hereby release, waive, discharge, and covenant not to sue Ty Long Kicking, its officers, employees, agents, representatives, successors, and assigns from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in these activities.
I further agree to indemnify and hold harmless Ty Long Kicking from any and all liabilities or claims made as a result of my participation in these activities, including but not limited to, liabilities arising from the negligence or fault of Ty Long Kicking, its officers, employees, or agents.
I understand that this waiver and release of liability will apply to all activities organized by Ty Long Kicking, including activities on the field and in the weight room.
I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights by signing it. I voluntarily agree to the terms of this waiver and release of liability.
Cancellation policy 

**Ty Long Kicking Cancellation Policy**

At Ty Long Kicking, we strive to provide the best training experience for our athletes. To ensure fairness and transparency, we have established the following cancellation policy:

### Monthly Training Subscription
1. **Notice Period**: 
   - To cancel your monthly training subscription, you must provide a minimum of two weeks' notice prior to the start of the next billing cycle.
   - Cancellations made with less than two weeks' notice will result in the billing for the upcoming month.

2. **How to Cancel**:
   - Cancellations must be submitted in writing via email to tylongkicking@gmail.com or through our website's contact form.
   - Please include your full name, contact information, and reason for cancellation in your request.

### Package Training
1. **Refund Eligibility**:
   - Refunds for package training sessions are only available in cases of injury.
   - A doctor's note and proof of injury are required to process the refund.

2. **Documentation Requirements**:
   - The doctor's note must include the nature of the injury, confirmation that the injury prevents participation in training, and the estimated duration of recovery.
   - Proof of injury can include medical reports, imaging results, or any other relevant documentation.

3. **Refund Process**:
   - Upon receipt of the required documentation, we will review your request.
   - Approved refunds will be processed within 14 business days and will be issued via the original method of payment.

4. **Non-Refundable**:
   - Please note that package training sessions are non-refundable for reasons other than verified injury.
   - Unused sessions cannot be carried over to another athlete or future date unless specified in writing and approved by Ty Long Kicking.
